Let me try again ... rDNS setup please?

I filed a request 3+ weeks ago:

And it still has not been setup. I have seen at least 2 others get theirs setup since my request and the requested info was submitted.

Can you please tell me if the hold up is on my end? If so, what information is missing or what isn't setup correctly on my end?
