I need a form filled out for Comcasts recent Address Remit Change. Who do I send it to?

In order to pay our invoice, we need to have the new remit address added to our system as is required by the Government Agency that oversees issuing payments. I am trying to figure out who I need to send the form to in order to remedy this issue so I can issue payment.
