Billing that still isn't corrected from May 2015

I have billing errors from May 2015 that still haven't been corrected. I have a ticket number of . I have been trying to fax paperwork for three days and it never goes thru. Give me somewhere to send the paperwork that it will actually get received and the accounting errors on your end fixed.  fax number does not ever work.
