voicemail on 1 phone has stopped working

I have deleted all old voicemails, thinking the user had exceeded the maximum.  That did not fix the problem.  When we transfer a caller to voicemail they hear the greeting and record their message but it never appears as a voicemail or email on the users phone.  It also does not appear on the portal.  The indicator light on the phone doesn't flash either.
